What is a PHO?
A PHO - Physician Hospital Organization is an integrated health care delivery system which combines the services of a hospitals, physicians and ancillary providers. Together the organization enters into contract agreements with managed care companies in order to provide medical care for patients. Cornerstone is a Messenger Model PHO that negotiates the non-economic terms of managed care agreements on behalf of our providers. There are currently 29 payers contracted with Cornerstone. How do I become a member of Cornerstone?
Membership is easy! Just click here for new provider applications that will help you being the membership process. Once the applications are completed, mail them Cornerstone Alliance, 71 Town Square, 2nd Floor, Lima, OH 45801 Attention: Cheryl Hawk, Credentialing Coordinator.
What is the initial membership fee for new providers?
The initial membership fee for new physician providers is $1500, AHP providers, $565 and for new ancillary providers, $565.
What is the benefit of being a member of Cornerstone?
Cornerstone was developed to simplify the managed care contracting and credentialing process of our network providers as well as reduce the providers’ costs associated with these services. Membership with Cornerstone allows access to 29 managed care contracts. For a list of contracted payers, click here.
As a member of Cornerstone, which contracts are available to me?
Providers are eligible to participate in all contracts. Certain restrictions apply to Mental Health and Substance Abuse providers, Chiropractic providers and Ancillary providers. If you are a provider of these services, contact Cheryl Hawk at 419-996-5316 for details on these restrictions.
As a member of Cornerstone, do I have to participate in all contracts?
Providers are not required to participate in all contracts.
