The pre-admission appointment is a very important part of having surgery or treatment at St. Rita's Medical Center. We want to be certain you are educated and feel informed about the surgery or treatment you are having done.

Please feel free to ask questions. This appointment is your opportunity to talk with trained, medical staff about your surgery or treatment and have it explained to you in simple terms you will understand.

Thank you for choosing St. Rita's Medical center for your healthcare. Our Pre-Admission Center will take information about your physical condition, find out your needs and see if you are ready for your surgery or treatment.

During your visit, we can find out any health problems that may be important to know for your surgery or treatment. By doing this before surgery, we can make sure that these problems are taken care of by your doctor.

What Should I Expect?
The following will happen at your pre-admission meeting:

  • The Date and Tim of Your Surgery/Treatment will be Checked
  • You will be Told Where and When to Check In the Day of Your Surgery/Treatment
  • Your Insurance Information will be Checked
  • Any Tests Your Doctor has Ordered will be Completed
  • Your Complete Medical History will be Taken
  • Instructions for Care Before/After the Surgery/Treatment will be Given
  • Your Questions will be Answered
Knowing what to expect and what to do is important in a successful surgery or treatment and getting well.

Your Pre-Admission Visit
The Pre-Admission Center will call you to set-up an appointment. When coming to your appointment:

  • Allow One to Two Hours for Your Appointment
  • Wear Comfortable Clothing
  • Bring Your Insurance Cards
  • Bring any Forms Your Doctor's Office Asks You to Fill Out
  • Bring a List of Times You Have Been to the Hospital
  • Bring a List of Any Allergies You Have
  • Bring a List of any Medications You are Currently Taking
  • Have a Family Member or Friend Come with You
If you have not been called within 48 hours of your surgery/treatment date, please call (800) 232-7762, extension 9652 or (419) 226-9652.

Everyone at St. Rita's is pleased that you have chosen use for your healthcare. The Pre-Admission Center is here to make sure your hospital stay is comfortable and pleasant.

Please feel free to ask any questions. The medical staff will be able to explain your surgery/treatment in terms you can understand. We care about you and want you to be well informed about what you are having done.

Financial & Insurance Information
If you have insurance, call the insurance company and make sure you are covered for this treatment before coming to your appointment. Some companies need a second opinion or other permissions before surgery/treatment.

Did You Know:
Patients may frequently hear medical staff using medical terms. One term, NPO, is a Latin term meaning nothing by mouth. Before surgery, certain lab tests and diagnostic tests, patients are required to take no food or water after midnight in preparation for the procedure or test. If you are unclear about pre-surgery or pre-test instructions, never hesitate to speak with a healthcare professional.
