Sports Walk-In Clinic

Our Sports Walk-In Clinic provides active individuals of all ages timely access to medical professionals specifically trained to evaluate and treat activity-related injuries. The Sports Walk-In Clinic is for those injuries/conditions incurred or that limit participation in sport, regular exercise, or recreational activities.

Certified Athletic Trainers (ATC)The St. Rita's Sports Walk-In Clinic is staffed by St. Rita's Certified Athletic Trainers (ATC). The ATC will evaluate the injury, discuss treatment options and provide a seamless transition for any injuries needing referral to supporting physicians. The supporting physicians provide us an immediate referral for appropriate testing and treatment.

The following information is offered to assist you in navigating our Sports Walk-in Clinic and to help manage your injury in a timely and efficient manner.

  • Individuals under the age of 18 require parent/guardian consent before treatment can be provided. Please accompany the athlete to the walk-in clinic. If parent/guardian can not be at the appointment, please send information on how you can be contacted by phone for verbal consent.
  • There is no charge for the ATC evaluation and recommendations.
  • If you are examined by the physician, you or your insurance will be billed for those services including, but not limited to, a physical exam, diagnostic tests (such as X-ray), or treatments (such as casting/splinting).
  • Insurance may require a referral from your Primary Care Physician (PCP) prior to treatment by another physician or specialist. Please verify insurance coverage prior to coming into the Sports Walk-in Clinic, as it helps ensure maximum benefit from your insurance plan.
  • Please have your insurance card available for us to make a copy.
  • If you suspect the injury involves the head, neck, or might be an internal injury, please inform the ATC immediately upon arrival. Head, neck or internal injuries may be referred to St. Rita's Medical Center Emergency Department, Putnam County Ambulatory Care Center or Delphos Ambulatory Care Center for treatment.
