Description   St. Rita's Medical Center in conjunction with the Neighborhood Nurses, offers a Lower Extremity Amputation Prevention program, (LEAP). The program is free and provides Blood Sugar Testing, Hemoglobin A1C, Urine testing, Foot and Nail Care, Diabetes Education, and Nutritional information. Locations may vary, so please contact Call A Nurse for location and to make an appointment at 419-226-9000 or 800-437-4827. Appointments will last approximately sixty minutes, if you attend the class and foot exam. If you are NOT a diabetic, please Fast for four hours prior arrival to your appointment.
